“Know Your Numbers” – Fall On-site Health Screenings and Flu Clinics

As part of the Wellness Your Way incentive program, full-time, benefits-eligible employees are invited to attend an on-site health screening which will count toward their 2019 Tier One Wellness Your Way requirements.

 

In addition, we have arranged flu vaccine clinics to align with all upcoming health screening dates through December 2018 for all campuses.    

 
Completing Your Tier One Activities

If your health screening is completed on-site with Be Well Solutions, ֱ State University's wellness partner, it will automatically be loaded into your personal portal. If your health screening is completed in your physician’s office, please log in to the Be Well portal, under “My Wellness,” select “Wellness Reward Program,” print the “TIER 1 – PHYSICIAN SCREENING FORM” and take it with you to your appointment. Your completed form should then be submitted to Be Well Solutions using the address on the form. 

The following on-site dates are now available to schedule your “Know Your Numbers” health screening and/or flu vaccine. Follow the “How to Schedule” instructions below as registration is required.

We will only be hosting one health screening and flu vaccine clinic per Regional Campus. Employees are welcome to attend any clinic that best fits their schedule. But don’t delay, register today!
 

How to Schedule Your Appointment

  • Once you are logged in, select “My Appointments,” then “Worksite Screening.” Choose the date and location that best meets your schedule. Please keep in mind that you will need to fast for 8 to 10 hours prior to your health screening and blood draw via a finger prick.
  • To complete your online health risk assessment while logged into Be Well, select “My Wellness” and then select “Health Risk Assessment.”
     
  • Results of your screening remain confidential at all times and will be provided to you by Be Well at the time of screening.
